Tryout waiver

CHICAGO - The UIC softball team is set to hold tryouts Thursday at 5:30 p.m. (CT) at Flames Field in Chicago, Ill.

In order to tryout for the Flames squad, each individual must bring a proof of a physical examination that was administered within the last year and is signed by a medical doctor. Additionally, all participants need to bring a copy of the front and back of her insurance card and all necessary softball equipment.

The signed waiver located at the top of this page is required to participate in the tryout. Forms are also available for pick up at the softball office, which is located in room 145 of the Physical Education Building.
